Making An Appointment

We accept patients through both physician referrals and self-referrals. Please call the office to make an appointment at 559-733-3346. If you are unable to keep your appointment, please let us know as soon as possible so we may use that time to see another patient in need of care.

Department of Motor Vehicles: You may qualify for a DP placard or DP license plates if you have impaired mobility due to having lost use of one or more lower extremities, or both hands, or have a diagnosed disease that substantially impairs or interferes with mobility, or one who is severely disabled to be unable to move without the aid of an assistive device.

Employment Development Department: The EDD manages the Unemployment Insurance (UI) and State Disability Insurance programs which include Disability Insurance (DI) and Paid Family Leave (PFL) for the State of California. You can file a claim for either of these programs online, by phone, or by mail.
